The Hague Convention forms were developed to comply with the Convention and reduce the complexity of international case processing for all Convention countries.
- Before sending a case to another Hague Convention country, U.S. states should check the current form and language requirements for the specific country on OCSE's Hague Convention Requirements page.
- The country’s Hague Country Profile also provides useful information.
- Unless these resources indicate otherwise, a U.S. state should always use the Hague forms for outgoing Hague cases. (See page 225 of the Practical Handbook for Caseworkers (PDF) for instructions on completing the forms.)
- A Hague Convention country sending a case to the U.S. should use the English-language forms.

| 4 | A2: Abstract of a Decision (PDF) | This form may accompany an application to recognize or to recognize and enforce a decision where the requested country has specified that it will accept an abstract of the child support decision. The form must be completed by the competent authority of the country of origin. |
| 5 | A3: Statement of Enforceability of a Decision (PDF) | This form may accompany an application to recognize or to recognize and enforce a decision and confirms that the decision is enforceable in the country of origin. The Statement of Enforceability is completed by the competent authority of the country of origin. |
| 6 | A4: Statement of Proper Notice (PDF) | This form may accompany an application to recognize or to recognize and enforce a decision and indicates that the respondent did not appear and was not represented in the proceedings in the country of origin but had proper notice of the proceedings and an opportunity to be heard or had proper notice of the decision and an opportunity to challenge it on fact and law. The Statement of Proper Notice is completed by the competent authority of the country of origin. |

| Request for Specific Measures | ||
| 15 | F1: Request for Specific Measures (PDF) | This form can be used to submit a request for cooperation to a Central Authority, when no Hague Convention application is yet pending. These requests are to assist in deciding whether to initiate a full application. Most commonly, these are parent locate requests. |
